How to get started in Physical Therapy?

- You will need a prescription from your doctor
- We will need your contact and insurance information
- Some insurances require prior authorization which we will need before we see your child for their first visit
- Schedule an appointment!
During your appointments, we need the child and the parent/guardian present. We will come to your home and treat your child in his/her natural environment. Our focus is to coach you in everything we are working on with your child. We are here to help your child AND your family!
What to have ready for your first appointment:
We have provided these documents below to have you fill out before we arrive to your home. If you do not have these ready, no problem! We can mail them to you at your request or we will bring copies for you to fill out during your appointment.
Please have copies of your child's previous medical records or therapy reports if you have them available.
